Slips, Trips, and Falls

Objectives:


Upon completion of this module, dialysis facility personnel will be able to:
  • recognize patient safety issues associated with slips, trips, and falls in the dialysis unit
  • increase awareness of patient safety issues associated with slips, trips, and falls
  • assess facility preparedness for slip, trip, and/or fall situations
  • assist patients with identifying potential slip, trip, and/or fall situations that can occur in the home setting and how they can minimize risks
  • identify best practices to prevent safety errors
  • engage patients and families to enhance their culture of safety


Activities Required for Diamond Status/Measures: 


In order to receive Diamond Status, your facility must meet the measures listed below: 
  • 100% of staff complete an in-service training module (PowerPoint Presentation) on slips, trips, and falls (available in English or Spanish)
  • Discuss fall prevention with patients, have 85% of patients sign the Patient Fall Prevention Education Form (available in English and Spanish), and have staff document discussions in medical record
  • Complete online Reporting Form
