Home Dialysis Therapy


Upon completion of this module, dialysis facility personnel will be able to:
  • educate all eligible patients about home dialysis therapy
  • assess all eligible patients for interest, and refer as appropriate, all eligible patients to home dialysis therapy
  • demonstrate an effective process for tracking referral outcomes, led by a Home Therapy Liaison
  • incoporate home dialysis therapy patient assessment, tracking, and referral process into facility's QAPI program
  • identify best practices to prevent safety errors
  • engage patients and families to enhance their culture of safety

Activities Required for Diamond Status/Measures: 

In order to receive Diamond Status, your facility must meet the measures listed below: 
  • 100% of staff complete the module pre-test (available in English or Spanish)
  • 100% of staff complete an in-service training module PowerPoint Presentation on home dialysis therapy (available in English or Spanish)
  • 100% of staff watch the Home Dialysis Simulation Training
  • 100% of staff complete the module post-test (available in English or Spanish)
  • For a period of no less than 3-months, track all new patients receiving home therapy AND that each patient has been appropriately referred to a home therapy program for a visit/appointment (you may use your own tracking mechanism or use the tool provided)
  • Review your facility's policies and procedures on home therapy education and referral for patients
  • Identify a healthcare worker in your dialysis facility to serve in the role of Home Therapy Liaison via online reporting form
  • Complete online Reporting Form

If your facility completes and submits this module as complete, all nurses who participated with their facility may apply for 1.0 continuing education credit for their participation. In order to receive continuing education credit: Credit will not be issued until after your facility has submitted the module's final step: the Culture of Safety Reporting Form, via the 5-Diamond Patient Safety Program website, and it has been marked as Complete by the 5-Diamond system.*

*If you encounter an issue with obtaining continuing education credit, please verify your facility has submitted the required module Reporting Form and that it has been marked Complete before contacting the National Office.
