Objectives:
Upon completion of this module, dialysis facility personnel will be able to:
recognize and implement patient-centered communication practices
demonstrate effective techniques that support patients with limited health literacy, cultural barriers, and/or low English proficiency
identify best practices to prevent safety errors
engage patients and families to enhance their culture of safety
Activities Required for Diamond Status/Measures:
In order to receive Diamond Status, your facility must meet the measures listed below:
100% of staff complete the in-service training module (PowerPoint Presentation) on health literacy (available in English or Spanish)
Complete Health Literacy Assessment with 25% random sample of patients and submit summary results (spreadsheet provided)
100% of staff review
8 Ways You Can Improve Health Communication
and sign staff log (available in English or Spanish)
Complete online Reporting Form
