About

The 5-Diamond Patient Safety Program was initially developed in 2008, as an 8-module collaborative between the Mid-Atlantic Renal Coalition (now Quality Insights Renal Network 5) and the ESRD Network of New England (formerly Network 1).

Over the years, the program has developed into an automated, online, national educational resource, consisting of 19 modules that include the tools and resources necessary to implement each patient safety concept. Any dialysis facility, regardless of Network area may view or access the program.  Modules may be completed for recognition or viewed as a resource.  For each module successfully completed during a program year,*  the facility earns one Diamond. Upon successful completion of five modules, and a Program Review questionnaire completed by all staff, the facility is recognized as a 5-Diamond Patient Safety Facility.

The program is endorsed by:
  • American Association of Kidney Patients (AAKP)
  • American Nephrology Nurses' Association (ANNA)
  • Dialysis Clinic Inc. (DCI)
  • Fresenius Medical Care (FMC)
  • Nephrology Nursing Certification Commission (NNCC)
  • Renal Healthcare Association [formerly National Renal Administrators Association (NRAA)]
  • Renal Physicians Association (RPA)
  • U.S. Renal Care

*In order for a module to be successfully completed, all module measures must be met.
