The ESRD Networks believe it is important for each dialysis facility to incorporate patient safety into its organizational culture. In 2008, the 5-Diamond Patient Safety Program was developed as an innovative training and recognition program to assist dialysis providers in increasing awareness and building a culture of safety among patients and staff. The 5-Diamond Patient Safety Program was created by the Mid-Atlantic Renal Coalition (MARC, ESRD Network 5) and the ESRD Network of New England (ESRD Network 1), is supported by the national ESRD Networks, and is endorsed by:

Getting Started...

In an effort to expand the reach of the 5-Diamond Patient Safety Program to those previously unable to participate, and for a smoother process for all participants, the program has been automated.  In order to view module tools & resources, or to complete modules for Diamond status, all visitors are asked to register.  Registration and participation are both free to any interested dialysis facility.

2015 Program Year
All modules must be submitted by 12/18/2015.

The 2016 Program Year will open 1/1/2016:  All current participants/viewers must re-register for access to the 2016 program materials. 

The NNCC recognizes 5-Diamond modules towards technician recertification requirements!
