The ESRD Networks believe it is important for each dialysis facility to incorporate patient safety into its organizational culture. In 2008, the 5-Diamond Patient Safety Program was developed as an innovative training and recognition program to assist dialysis providers in increasing awareness and building a culture of safety among patients and staff. The 5-Diamond Patient Safety Program was created by the Mid-Atlantic Renal Coalition (MARC, ESRD Network 5) and the former Network of New England, is supported by the national ESRD Networks, and is endorsed by:
